I'm also one of the lucky ones who lacks an intermediate number. I was deciding between a 42 and a 43. The 42 was just too tight without the slightest clearance. In the end I took the 43 despite the fact that they were a little looser in the front. A special insert solved it. Now they fit perfectly. In winter, I will use the original insole for thicker socks. For the given price, I can only recommend, really good sneakers. And they look good.
