Military test pilots coined the phrase in the 1960s as they tested aircraft to their limits, risking their lives to the brink. They pushed their ball-on-top throttles all the way forward against the cockpit wall to achieve maximum speed and used the phrase: Balls to the Wall.
The BTW suit is precisely designed for such races, when you never shift up from a 53 × 11 or 12 gear and you go at your limit, or in corners at the limit of adhesion. When everyone is driving at the limit of risk.
